To obtain and maintain status as a Peoples Health contracted provider, the provider must meet qualifications including but not limited to the following:
- Have a current, valid, unrestricted Louisiana state medical or professional license or registration certificate(s) (there must not be any restrictions on the provider’s license to practice medicine)
- Have a current, valid, unrestricted Drug Enforcement Agency certificate or current, valid, unrestricted Louisiana Controlled Dangerous Substance certificate if applicable
- Meet accreditation standards*
- Provide requested information, including but not limited to the following, for review by the Peoples Health credentialing department:
- Peer references
- Work history
- Malpractice claims history
- Proof of liability insurance coverage
- Maintain status as a Medicare provider and not be on the Medicare Preclusion List
- Adhere to Quality Improvement and Medical Management program requirements; refer to Section B.7 (Provider Office Quality Standards) for more information
