Based on CMS guidance, we will check with you at least quarterly to find out if there are any changes to your demographic information. Maintaining accurate provider information is essential to ensuring that our plan members can access covered services and are able to make informed decisions about their health care.
If your office or facility demographic details—including address, phone number, hours of operation, ability to accept new patients or general availability—change, it is your responsibility to inform Peoples Health. Please update the information using the online Provider Profile Verification link in Member Viewer, accessible through Provider Portal. You may also download the Provider Information Change Form from www.peopleshealth.com/for_providers/update-provider-information, and complete and fax it to 1-888-346-5702.
If your demographic details have not changed, there’s no need to take action. Thank you for helping us keep our records up to date. Contact your Peoples Health representative with any questions.
