For your Peoples Health patients to utilize their plan benefits in the most cost-efficient way, it is essential that they know which providers belong to their plan’s provider network. We use various methods to educate and inform our plan members and the general public about the identity of our providers.
Peoples Health shall have the right to include the following information for contracted providers in any and all marketing and administrative materials it distributes:
- Provider name
- Business telephone number
- Business address
- Business hours of operation
- Type of practice or specialty
- Hospital accreditation status
- Hospital quality data from recognized sources
Please make any updates to your information using the online Provider Profile Verification link in Member Viewer, accessible through Provider Portal. You may also download the Provider Information Change Form from www.peopleshealth.com/for_providers/update-provider-information, and complete and fax it to 1-888-346-5702.
