CMS implemented the Medication Error Identification and Reduction system to identify and reduce medication errors. If any of your Peoples Health patients have experienced a problem at a pharmacy with a medication purchase, call 1-866-553-5705 and include the following information:
- The patient’s name
- The patient’s member ID
- The patient’s contact phone number
- Date of the issue
- A description of the issue, such as:
- Incorrect drug
- Incorrect strength
- Incorrect directions on label
- Incorrect patient name on label
- Dispensed prescription to the wrong patient
- Dispensed expired medication
- Incorrect drug preparation
- The dispensing pharmacy
- If the error has been corrected
- Any additional information or comments from you or the patient
Our clinical pharmacists collect and review errors to identify trends and patterns. Please note: This tracking process is in addition to any existing complaint or grievance rights your patient has as a Peoples Health plan member.
