Medication Error Identification and Reduction System

CMS implemented the Medication Error Identification and Reduction system to identify and reduce medication errors. If any of your Peoples Health patients have experienced a problem at a pharmacy with a medication purchase, call 1-866-553-5705 and include the following information:

  • The patient’s name
  • The patient’s member ID
  • The patient’s contact phone number
  • Date of the issue
  • A description of the issue, such as:
    • Incorrect drug
    • Incorrect strength
    • Incorrect directions on label
    • Incorrect patient name on label
    • Dispensed prescription to the wrong patient
    • Dispensed expired medication
    • Incorrect drug preparation
  • The dispensing pharmacy
  • If the error has been corrected
  • Any additional information or comments from you or the patient

Our clinical pharmacists collect and review errors to identify trends and patterns. Please note: This tracking process is in addition to any existing complaint or grievance rights your patient has as a Peoples Health plan member.
